Students must have a media permissions form on file at school to be interviewed or included in pictures and/or videos at SCHS.
Athletics
The Media Permissions Form is a required form that gives permission for students to be pictured, interviewed and/or published.
The Athletic Drug Consent Form is a required form for athletes and parking permits.

Attendance
Educational Enhancement Opportunity (EHO) request form must be completed and turned in 5 days before the requested absence.
Code of Conduct
Scott County Schools Student Discipline Code of Acceptable Behavior for the 2022-2023 School year. Students are required to sign and turn in the last page.
Parking
To Park at SCHS, students must turn in a completed parking permit application, which includes a drug test consent form. See the SCS Drug Testing Policy here.
Technology
There are several required technology forms. Students must have a completed AUP form to access technology at SCHS. See the Acceptable Use Policy here.
The Pledge for Device Use and Device Protection forms are both required to receive a SCS Chromebook.
